Job summary

Interim HealthCare of the Black Hills is seeking a compassionate Home Health Registered Nurse (RN) Case Manager for part-time roles across Spearfish and nearby communities. You will develop personalized care plans, coordinate with an interdisciplinary team, and support patients to recover at home.

The ideal candidate is a licensed RN in South Dakota with strong communication skills, the ability to work independently, and a passion for patient-centered home health care.

Qualifications

• Must be a licensed RN in South Dakota.
• Passionate about patient-centered care.
• Organized, self-motivated, and able to work independently.
• Excellent communication and critical thinking skills.
• Experience with electronic medical records and clinical documentation.
• Willing to work as part of a collaborative team and commit to continuous learning.
• Home health experience preferred but not required.

Responsibilities

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Manage a caseload of home health patients from admission through discharge.
• Coordinate care with physicians, therapists, social workers, home health aides, and other team members.
• Provide skilled nursing visits, education, and disease management.
• Complete accurate and timely OASIS assessments and clinical documentation.
• Educate patients and families to promote independence and confidence in managing their health.
• Monitor patient progress and adjust care plans as needed.
• Advocate for patients while ensuring quality, safety, and compassion.
• Participate in case conferences and collaborate with the Interdisciplinary team.
