Responsibilities

• Coordinate individualized care plans as a Home Health Case Manager.
• Conduct comprehensive assessments and develop tailored treatment strategies.
• Collaborate with patients, families, and interdisciplinary teams to achieve optimal outcomes.
• Document care accurately, including progress notes and discharge summaries.
• Educate patients and caregivers to promote safe, confident transitions.
