Job summary

Phoenix Home Care in Bolivar, MO is seeking a skilled Home Health Case Manager to coordinate patient care in the home, supervise LPN visits, and collaborate with PT/OT/Speech. You will perform assessments, interpret diagnostics, and develop plans aligned with physician orders and initial evaluations.

Missouri or Compact RN licensure and a valid driver’s license are required; home health or critical care experience is preferred.

Qualifications

• Missouri or Compact RN license required.
• Driver's license valid.
• Home Health or Critical Care experience preferred.

Responsibilities

• Caseload management including supervision of LPN visits.
• Weekly caseload includes visits, evaluations, and starts of care.
• Educate patients on conditions (CHF, COPD, Diabetes) and medication management.
• Communicate with physicians to obtain orders and write plans of care.
• Collaborate with PT/OT/Speech therapy teams.
