Job summary

Advocate Health seeks a Nurse Case Manager to cover the Milwaukee Metro area with a 4-day workweek from 10:00 to 20:00. You will manage caseloads in specialty areas, perform comprehensive assessments, and develop patient-centered care plans in collaboration with physicians and the health team.

You will provide direct nursing care, document thoroughly, and participate in discharge planning and quality improvement activities. A 3-year nursing background and state RN license are required.

Qualifications

• Registered Nurse license in the state of practice.
• BLS for Healthcare Providers certification within 6 months.
• Valid driver’s license and a registered, insured vehicle.
• Associate’s Degree in Nursing.
• Typically 3 years of clinical nursing experience.

Responsibilities

• Assumes responsibility for caseloads in specialty areas.
• Conducts initial assessment to obtain thorough physical, social and environmental data.
• Establishes the plan of care per physician orders and patient needs.
• Provides direct patient care and adjusts nursing care as needed; collaborates with family and health team.
• Performs ongoing patient assessments and revises care plan as needed.
• Completes documentation including visit assessments and orders promptly; maintains records.
• Communicates with physicians to update patient plans and ensure continuity of care.
• Participates in discharge planning and post-discharge coordination.
• Participates in chart audits and quality improvement activities.
• Maintains accurate clinical and patient records per guidelines.
• Demonstrates age-appropriate care and knowledge of growth and development.
