Job summary

Advocate Health in Wisconsin seeks a Registered Nurse to join the Milwaukee North 4 - Nursing team. You will manage caseloads, perform assessments, and establish care plans in coordination with physicians and the care team.

The role is full-time with a 40-hour work week, Monday–Friday 8:00–4:30, offering competitive pay and a comprehensive benefits package. A valid RN license and Associate’s Degree in Nursing are required.

Qualifications

• Registered Nurse license issued by the state of practice (or eligible)
• BLS for Healthcare Providers certification within 6 months
• Associate's Degree in Nursing

Responsibilities

• Assumes responsibility for caseloads in specialty areas.
• Conducts initial assessment to obtain thorough physical, social and environmental data.
• Establishes the plan of care in accordance with physician orders and patient needs.
• Provides direct patient care and updates nursing care plan as needed; communicates with family and team.
• Monitors patient response to treatments and revises plan as indicated.
• Completes documentation including visit assessment and orders.
• Collaborates with physicians to update the plan of care and ensures continuity of care after discharge.
• Participates in chart audits and quality improvement initiatives.
