Job summary

Optum Home & Community Care in Seattle seeks an RN for field-based home visits to assess health status and guide care plans. You will identify gaps, provide patient education, and refer to resources to support self-management.

In this high-volume, customer-service environment, you will navigate medical software and maintain strong patient-focused communication while coordinating with care teams and family members. Requires WA RN license, BLS, and reliable transportation.

Qualifications

• Current, unrestricted RN license in Washington state.
• BLS certification or ability to obtain by start date.
• 1+ years of clinical experience.
• Experience in home care/home visits.
• Reside in or near Seattle, WA.
• Driver's license and reliable transportation.

Responsibilities

• Visit members in their homes to assess their current health status.
• Identify gaps or barriers in treatment plans.
• Provide patient education to assist with self-management.
• Make referrals to outside sources.
• Provide a complete continuum of quality care through close communication with members via in-person in home or on-phone interaction.
• Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels.
