Job summary

AccentCare, Inc. is seeking an RN Case Manager for Home Health in Sugar Land, TX. This full-time role focuses on coordinating patient care from admission to discharge, guiding staff, and ensuring quality outcomes in a home health setting.

The candidate must be a licensed RN with CPR certification and at least one year of RN experience, with ability to manage detailed care plans and collaborate with interdisciplinary teams. On-site position with office in Sugar Land.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of operation.
• Minimum of one year of experience as an RN.
• Current CPR Certification required.

Responsibilities

• Assess the status of nursing needs and manage the patient’s care plan.
• Complete initial patient assessment upon admission and document history.
• Select appropriate nursing diagnoses and monitor clinical effects of medications.
• Communicate results and recommended interventions to physicians and team.
• Implement nursing interventions within the Nurse Practice Act.
• Re-evaluate patient needs and adjust plans as required.
• Document assessment, progress, and discharge notes timely.
• Inform physicians of changes in patient condition and care needs.
