Job summary

AccentCare, Inc. is seeking an RN Case Manager for Home Health in Katy. The role manages care plans from admission to discharge, coordinating with nurses, LPNs and CHHAs to ensure quality patient care.

The position requires Texas RN licensure, CPR certification, and at least 1 year of nursing experience. Salary range is $83,000 to $102,000 per year, with a Monday–Friday schedule and on-call responsibilities.

Qualifications

• RN license in Texas required.
• CPR certification required.
• Minimum of 1 year of RN experience.

Responsibilities

• Assess the status of nursing needs and manage the patient's care plan from admission through discharge.
• Complete initial patient assessment and documentation.
• Identify nursing diagnoses and monitor therapy for adverse effects.
• Coordinate with physician and other clinicians to implement care plans and meet goals.
• Re-evaluate patient needs and adjust plans as required.
