Amedisys is seeking a Registered Nurse case manager for full-time day shifts in Massachusetts. The territory covers Leominster, Fitchburg, Lunenburg, Westminster, Princeton, Lancaster, Sterling, Holden, Clinton, Boylston and West Boylston, delivering in-home care and hospice services to patients at home.

Qualifications

• One (1+) year of clinical RN experience.

Responsabilités

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
