Job summary

Eden Capital Careers is seeking an experienced Home Health RN Case Manager for a field-based role serving Dallas/Fort Worth and the surrounding Tarrant County area. The ideal candidate will manage a patient caseload in the home, perform skilled nursing visits, assessments, and coordinate care with physicians and families.

Must hold an Active Texas RN license with recent home health case management experience, strong assessment skills, and solid documentation habits.

Qualifications

• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.

Territory: Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville

We are seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.
