UnitedHealth Group, via its Amedisys affiliation, seeks a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate discharge planning, and support ongoing outpatient care.

You will work under supervision of an RN or MD to ensure appropriate care levels, provide education, coaching, and treatment-decision support, and collaborate with a Care Advocate and Social Worker.

Formación

• Current RN license valid in the state(s) you are assigned to work.
• CPR certification is required.
• 1+ year of clinical RN experience.
• Valid driver's license with reliable transportation and liability insurance.

Responsabilidades

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitative measures; educates on self-care techniques.
