Safe Life Home Health Care, LLC is seeking a Home Health Registered Nurse Case Manager in Rochelle, IL to join our 4-star team. The role emphasizes preventing hospitalizations and delivering compassionate in-home care, with a minimum of 1 year home health experience and proficiency in WellSky Kinnser EMR.
You will conduct thorough clinical assessments, coordinate with physicians, and support patients and families in their homes.
Competenze
- Valid Illinois RN license.
- Valid CPR Certification.
- Valid Driver's License.
- Valid Auto Insurance.
- Minimum 1 year home health experience.
- Proficiency in WellSky Kinnser EMR.
- Strong clinical assessment and communication skills.
- Compassionate and patient-centered approach.
