Job summary

Home Health Care, Inc. is seeking an experienced Registered Nurse (RN) Case Manager to deliver high-quality home health care across Minnesota.

This full-time role offers a balanced schedule with weekdays and a monthly on-call shift, supporting patients one-on-one in their homes. You will complete OASIS assessments, coordinate with physicians and the care team, and educate patients to promote safety and independence.

Qualifications

• Active Minnesota RN license required.
• 3+ years RN experience (home health preferred).
• Strong clinical judgment and ability to work independently.
• Excellent organization and communication skills.
• Reliable transportation for travel across territory.
• Must be able to complete and coordinate care plans with clinicians.

Responsibilities

• Manage a home health caseload across Woodbury, Hastings, and surrounding areas.
• Complete OASIS assessments and individualized plans of care.
• Provide skilled nursing visits, assessments, and patient education.
• Coordinate care with physicians and the interdisciplinary team.
• Help patients remain safe and independent in their homes.
• Maintain accurate and timely clinical documentation.

Registered Nurse (RN) Case Manager – Home Health | Reclaim Your Work-Life Balance

Schedule: Full-Time | Monday-Friday, 8:00 AM - 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week

Compensation: $85,000 - $130,000 annually plus $6,000 in first-year bonuses
