Job summary

Jobtailor is seeking an experienced Registered Nurse to manage patient caseloads, conduct thorough assessments, and develop and adjust plans of care in collaboration with physicians and families. The role includes direct patient care, discharge planning, and ongoing chart documentation to ensure continuity of care.

Qualifications

• Associate's Degree in Nursing is required.
• RN license issued by the state of practice is required.
• BLS for Healthcare Providers through the American Heart Association must be obtained within 6 months unless exempt.
• Valid driver license with insured vehicle.
• Typically requires 3 years of clinical nursing experience.
• Excellent communication and interpersonal skills.
• Ability to work with physicians and other professionals.
• Proficiency in clinical skills and ability to work under direction and make sound judgments.

Responsibilities

• Manage caseloads of patients in specialty areas.
• Perform initial assessments to gather physical, social, and environmental data.
• Establish care plans per physician orders and patient needs.
• Provide direct patient care and adjust nursing processes as needed.
• Identify problems, modify care plans, and evaluate outcomes.
• Consult with families and other healthcare personnel.
• Monitor patient responses to treatments and teaching; apply interventions.
• Discharge planning and ensuring continuity of care after discharge.
• Assist in chart audits and quality-improvement activities.
• Maintain accurate patient records per guidelines.
