Job summary

Qualifications

• Current licensure by the State Board of Nursing in Illinois or Michigan.
• Current American Heart Association HealthCare Provider BLS is required prior to start date.
• Own transportation and valid driver's license.

Responsibilities

• Provide patient-centered home health care across all ages from infancy to geriatric.
• Coordinate inter-disciplinary care with nursing, therapy, social work and home infusion teams.
• Manage visit planning, episode management and outcome monitoring in the home setting.
