Phoenix Home Care & Hospice in Overland Park, KS is seeking a Home Health Case Manager to coordinate patient care in home settings and supervise visiting nurses. You will assess patients, interpret diagnostics, and develop care plans aligned with physician orders while collaborating with PT/OT/Speech and other clinicians.

Qualifications

• Must hold Kansas or a Compact RN license.
• Driver's license and vehicle with current auto insurance.
• Home Health or Critical Care experience preferred.

Responsabilités

• Manage caseload including supervision of LPN visits.
• Weekly caseloads include routine visits, evaluations and initial starts of care.
• Educate patients on health conditions such as CHF, COPD, diabetes, wound care, and medication management.
• Communicate with physicians to obtain orders and write diagnosis-driven care plans.
• Collaborate with PT/OT/Speech and other therapy teams.
