Zusammenfassung

DayToDay Health is seeking an experienced Registered Nurse to provide individualized home health nursing care in the community. You will perform patient assessments, develop and coordinate personalized care plans, and deliver skilled nursing interventions based on clinical needs.

You will respond to changes in patient condition, educate patients and caregivers, coordinate services with an interdisciplinary team, and maintain accurate clinical documentation in line with care standards.

Qualifikationen

• Registered Nursing license required.
• Strong clinical assessment and critical thinking skills.
• Excellent communication abilities.
• Ability to work independently in a home setting while partnering with multidisciplinary teams.
