Job summary

Health Care Administrators Association® (HCAA) is seeking a Home Health Nurse (Case Management) to manage a patient caseload in home settings. You will perform patient assessments, develop and oversee individualized plans of care, and coordinate multidisciplinary services to ensure continued, appropriate treatment.

You will supervise LPNs and aides, communicate with care teams, and evaluate outcomes to ensure patient-centered goals are met.

Qualifications
