Job summary

James River Home Health and Hospice seeks a compassionate RN Case Manager to deliver home health nursing services in patients’ homes, overseeing care plans, and collaborating with the interdisciplinary team to maintain high-quality outcomes.

The role requires a VA RN license, with 1+ year home health experience preferred, a valid driver's license, and a reliable car. Excellent communication and documentation in Homecare Homebase are essential.

Qualifications

• Must be a Registered Nurse with Virginia license.
• At least 1 year of home health experience preferred.
• Graduate of an accredited nursing program.
• Valid driver’s license and reliable car required.
• OASIS and HCHB experience preferred.

Responsibilities

• Provides skilled nursing care to homebound patients per Plan of Care and develops individualized treatment plans.
• Coordinates and manages multidisciplinary services, admissions, transfers and discharges with the team.
• Educates patients and families on health maintenance and disease prevention; documents in Homecare Homebase.

RN Case Manager, Home Health

James River Home Health is seeking a compassionate and dedicated Home Health RN Case Manager to join our team and make a meaningful difference in the lives of patients receiving care in the comfort of their own homes.

In this role, you will manage a caseload of patients in their homes while collaborating with the interdisciplinary team to ensure a seamless, high-quality care experience.

Salary Range: $73,000 - $90,000

Territories: We currently have two openings, one in our South Territory and one in our North Territory

A brief overview of Responsibilities:

• Provides skilled nursing care to homebound patients in accordance with the established Plan of Care, developing and implementing individualized treatment plans while monitoring and reporting changes in condition.
• Coordinates and manages multidisciplinary services to ensure continuity of care, including admissions, transfers, and discharges, and collaborates closely with the interdisciplinary team.
• Educates patients and families on health maintenance and disease prevention, completes timely electronic documentation in Homecare Homebase, and offers guidance and emotional support throughout the care journey.
  

Qualifications:

• Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
• At least one year of home health experience preferred
• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable car available during work hours.
• OASIS and HCHB experience PLUS!
