Job summary

Artemis is seeking a Registered Nurse (RN) for home health in Salem, OR. The role covers Salem and surrounding areas, focusing on in-home assessments, care planning, and coordination with families and physicians.

The ideal candidate holds an Oregon RN license, either an Associate’s or Bachelor’s degree in Nursing, at least 1 year of acute care experience, BLS and CPR certification, and a valid driver's license with insured transportation.

Qualifications

• Active Oregon RN license.
• Associate or Bachelor’s degree in Nursing.
• Minimum of 1 year of recent acute care nursing experience.
• Current BLS and hands-on CPR certification.
• Valid driver’s license with reliable, insured transportation.

Responsibilities

• Complete comprehensive in-home nursing assessments and identify each patient’s clinical and ongoing care needs.
• Develop, implement, and update individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team.
• Provide skilled nursing care, education, and support to patients and caregivers in the home setting.
• Monitor changes in patient condition and communicate updates or concerns to the appropriate members of the care team.
• Coordinate services and assist with discharge planning to support safe transitions throughout the patient’s episode of care.
• Maintain accurate and timely clinical documentation while independently managing an assigned territory and caseload.
