Job summary

AW Health Care is seeking a Registered Nurse to perform the initial home evaluation and provide skilled nursing care in the patient’s residence. You will implement physician orders, address clinical needs, and collaborate with the care team to ensure high-quality home health services.

The role requires Missouri RN licensure and valid driver's license; prior hospital experience is preferred. You will document care and coordinate with Home Care Aides while maintaining patient safety and comfort.

Qualifications

• Graduate of an accredited nursing program with active RN license in Missouri.
• One year of acute hospital nursing experience preferred.
• Knowledge of home health care principles, safety, and infection control.

Responsibilities

• Makes initial evaluation visit to the patient’s home.
• Provides nursing services and treatments per physician plan.
• Implements care plan and preventive nursing procedures.
• Assess, educate, counsel, and provide nursing care to the patient.
• Maintain and write complete progress notes and patient records.
