Qualifications

Active RN license in the state you work.
One year of RN experience and the clinical expertise to deliver quality patient care.
A valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations.

Responsibilities

Deliver skilled, patient-centered care in home health settings.
Perform patient assessments, medication management, and care planning.
Educate and empower patients and families on disease management and treatment plans.
Think critically, solve problems in real time, and ensure thorough EMR documentation.
Work independently, but collaborate with a broader team.
Travel to patient homes within assigned territory.
