Job summary

Louisiana HomeCare, a part of LHC Group within the Optum family, delivers nursing care at patients' homes. The RN in Home Health coordinates plan of care with physicians and families to ensure continuity and quality. Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to dependable transportation.
• LA state requirements: 1+ year RN experience; waived if recent RN/LPN experience.
• Preferred CPR certification; 1+ year home health experience; ability to work independently.

Responsibilities

• Initiates and revises care plans in collaboration with physicians and health professionals.
• Supervises home health aides and LPNs; provides instruction and assigns tasks.
• Documents visits, completes assessments, and ensures quality, safe patient care.
• Participates in on-call and weekend rotation; supports survey readiness and improvements.
