Qualifikationen

• Active RN license in the state you work
• Minimum of 1 year RN experience
• Valid driver's license and ability to spend approximately 20% of your workday driving to and from patient locations are required

Aufgaben

• Deliver skilled, patient-centered care in patients' homes in collaboration with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Think critically, solve problems in real time, and ensure thorough EMR documentation
• Work independently, but remain connected with the team
• Travel to patient homes within assigned territory
