Job summary

AHN Healthcare@Home is seeking a Registered Nurse (RN) to provide skilled nursing care to patients in their homes in the Monongahela, PA area. This role offers a $10,000 first year success bonus and opportunities for ongoing rewards as you grow with the company.

You will deliver patient-centered care, perform assessments, educate families, and document thoroughly in the EMR. Travel to patient homes within your territory is required, with support from a dedicated education team and leadership

Qualifications

• Active RN license in the state you work.
• Minimum one year of RN experience.
• Current driver’s license and ability to drive ~20% of the day.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.
• Think critically and ensure thorough EMR documentation.
• Work independently, but collaborate with the team.
• Travel to patient homes within assigned territory.
