Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) for Home Health in the Carlisle/Plainfield area. The role is full-time (Staff) with a typical 8-hour shift, 40 hours per week, and requires travel to patient homes within the assigned territory.

You will deliver skilled nursing care in patients’ homes, perform assessments, manage medications, and educate families on disease management. A $10,000 first-year bonus recognizes achievement milestones during the first year.

Qualifications

• Active RN license in the state you work
• One year RN experience
• Must have valid driver’s license and be able to drive ~20% of workday to patient locations

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Think critically, ensure thorough EMR documentation
• Work independently but collaboratively with the team
• Travel to patient homes within assigned territory
