Job summary

Qualifications

• Must hold a current RN license.
• Preferably have at least one year of clinical experience.
• Familiarity with HCHB EMR is a plus.

Responsibilities

• Follow care plans according to patient needs.
• Make home visits in designated areas.
• Administer medications and perform wound care.
• Monitor client conditions and report changes.
• Document doctors' orders and client assessments.
• Provide case management and care coordination.

Skills

Current State or Compact Registered Nurse (RN) License

Clinical experience

Knowledge of current nursing practice

Valid state driver's license

Knowledge of HCHB EMR
