The role involves coordinating patient care and assessing individual needs, requiring 1 year of Home Health experience and knowledge of OASIS. Qualifications

• Must have Home Health experience and knowledge of OASIS.
• Graduated from an accredited registered nurse program.
• Licensed as a registered nurse in the state of practice.
• Current CPR certification is required.
• 1 year of Home Health experience is required.

Responsibilities

• Obtain data and assess necessary patient information.
• Coordinate care with interdisciplinary team.
• Re-assess patient needs and update the plan of care.
• Perform therapeutic, preventative, and rehabilitative nursing procedures.
• Maintain accurate patient records.
• Supervise and document delivery of care by the Home Health Aide.
