Pathways Healthcare is a leading home health and hospice provider focused on patient-centered care in the Kennebunk area. The Registered Nurse – Home Health role offers a meaningful opportunity to deliver high-quality nursing care in patients' homes, with a schedule Monday through Friday and a competitive annual salary.

Ideal candidates have 2+ years of nursing experience, especially in geriatrics. Maine RN licensure is required, with NH licensure preferred; new graduates are encouraged to apply

Qualifications

• 2+ years' nursing experience, preferably in geriatrics.
• New nurses welcomed; training program available.
• Degree in Nursing from an accredited institution.
• Active license in the state of Maine required
• Active license in the state of New Hampshire also preferred
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.

Responsibilities

• Assesses and evaluates patient's status by writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan with goals based on nursing diagnosis and family involvement.
• Initiates preventive and rehabilitative nursing procedures; administers prescribed medications.
• Completes and submits accurate clinical notes; tracks pain/symptom changes.
• Communicates with physician regarding patient needs and orders.
• Provides and maintains a safe patient environment.
• Attends interdisciplinary group meetings.
