Phoenix Home Care & Hospice is seeking a Weekend On-Call Registered Nurse to serve Overland Park, KS and surrounding areas. You will be the first point of contact between a patient and hospital, formulating an individualized plan of care with physician orders and coordinating with a multi-disciplinary team.

Responsibilities include developing and revising the care plan, monitoring patient conditions, and providing ongoing instruction to patients and families.

• Strong skills in physical assessment, diagnostic interpretation and communication to formulate an individualized plan of care.

Responsabilités

• Assists in development, review and revision of the plan of care.
• Provides nursing care per physician’s plan and client’s plan of care as authorized.
• Observes, records and reports reactions to treatment and changes in client condition to appropriate personnel and/or physician.
• Provides ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment.
• Participates in education programs, quality assessment activities, nurse’s meetings and other staff meetings as required.
• Monitors and records patient’s condition and documents provided care via electronic charting.

As a Phoenix RN, you will be the first point of contact between a patient and hospital. Our Home Health RNs should possess strong skills in physical assessment, diagnostic interpretation, and communication to formulate an individualized plan of care according to physician orders that incorporates the analysis of initial assessment. Working alongside other nurses, Physical Therapist, Speech Therapist and or Occupational Therapist.

Responsibilities

Requirements

• Current Registered Nurse license in Kansas or Compact.
• Home Health or Critical Care Preferred.
• Driver's License and Vehicle with Current Auto Insurance
• Ability to Pass Drug Test, Background Check, and Physical
