Job summary

Cynet Health is seeking a Registered Nurse with home health expertise to perform patient visits across assigned areas. You will work with HomeCare HomeBase EMR and OASIS to document care, ensuring timely entries within 24 hours of visits.

The role supports a typical daily caseload of 5–8 patients and weekly productivity targets. You must be willing to travel up to 75 miles, with mileage reimbursed. Open shifts and rotations are required.

Qualifications

• Active RN license required with strong home health experience.
• Ability to perform home visits and manage patient documentation efficiently.
• Proficiency with HomeCare HomeBase EMR and OASIS required.

Responsibilities

• Provide home visits to patients in their homes.
• Maintain 24-hour documentation following visits; complete on time.
• Manage a daily caseload of 5–8 patients and 28–30 units weekly.
• Travel up to 75 miles from clinic; mileage reimbursed at IRS rate.
• Open availability including rotations; travel reimbursements apply.
