Qualifications

• Active RN license in Ohio.
• BSN preferred but not required.
• 2+ years of home health nursing experience.
• Strong clinical assessment and critical thinking skills.

Responsibilities

• Conduct initial and ongoing assessments in patients' homes.
• Develop, implement, and evaluate plans of care.
• Administer medications, wound care, and treatments.
• Educate patients and families on care routines and disease management.
• Collaborate with physicians, therapists, and the interdisciplinary team.
• Document care accurately in EMR and ensure compliance with standards.
• Participate in weekend rotation as needed.
