Lorian Health is seeking a Home Health RN in California to provide skilled nursing care in patients’ homes according to their Plan of Care. The role involves patient assessment, medication administration, education, and meticulous documentation.

The position offers a pay-per-visit model ranging from $100 to $160 per visit, with on-call weekends. A BSN is preferred, RN license required, and a valid driver’s license is essential.

Competenze

• Graduate of an accredited school of professional nursing; BSN preferred.
• Currently licensed to practice as an RN in the state of practice.
• IV or ET certification, if held, recognized by the state(s) where practicing.
• Minimum of one year of recent Home Health or Hospice experience (within last 3 years).
• Possess and maintain a valid driver’s license.

Mansioni

• Provide skilled nursing care per the nursing process and patient Plan of Care.
• Assess and monitor patient condition and re-evaluate needs.
• Administer medications, treatments and procedures within scope and competency.
• Communicate changes in patient condition with physicians, supervisors, patients and families.
• Educate patients and families on meds, treatment plans and disease management.
• Complete accurate clinical documentation per Lorian policies and regulatory requirements.
• Promote safe care environment and support activities of daily living.
