Job summary

Trinity Health At Home is seeking a Home Care RN to provide one-to-one nursing in patients' homes across New Castle County, Delaware. This full-time Day Shift role offers a $10K sign-on bonus and a patient-centered care model with strong support from a collaborative team.

Responsibilities include initial assessments, care-plan development with the Population Health Case Manager, and supervision of aides and LPNs as needed. A current Delaware RN license is required.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the state of Delaware.
• One year of professional nursing experience.
• Acute Care or Home Care experience preferred.
• Valid driver's license and reliable transportation.
• Ability to work weekend rotations and holidays.
• No on-call duties.

Responsibilities

• Provide primary nursing care in patients' homes as ordered by physician.
• Develop initial care plan in collaboration with Population Health Case Manager.
• Supervise Home Healthcare Aides and LPNs as needed.
• Deliver therapeutic interventions and overall care management.
