Job summary

Trinity Health Of New England At Home in the Enfield/Windsor region seeks a Home Health RN to deliver skilled nursing care at patients' homes. You will create and execute individualized care plans, administer medications, and educate families to support recovery and independence.

Qualifications

• Graduate of an approved nursing education program; BSN preferred.
• Active Connecticut RN license and BLS Certification.
• One year of professional acute care nursing experience.
• Home health experience preferred but not required.

Responsibilities

• Deliver skilled nursing care in patients' homes with individualized care plans.
• Administer medications and educate patients and families.
• Support recovery and independence with 1:1 patient interactions.
