Job summary

TRINITY HEALTH OF NEW ENGLAND AT HOME is seeking a Home Care RN to deliver primary nursing care in patients' homes under physician orders, with initial assessments and care planning in collaboration with the Population Health Case Manager.

You will supervise aides and LPNs as needed, coordinate with the care team, and follow policy. The role includes a $10,000 sign-on bonus and day-one benefits. Schedule: Full-Time, Mon-Fri 8am-5pm with weekend rotation and two holidays.

Qualifications

• Graduate of an approved nursing program; BSN preferred.
• Licensed as a Registered Nurse in Connecticut.
• One year of professional acute care nursing experience.
• Home care experience preferred but not required.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess patient condition and risk at admission and as needed.
• Collaborate with physician and care team to develop a plan of care.
• promptly report changes in patient condition to the care manager.
• Follow the plan of care and utilize guidelines and education tools.
