Residential Home Health and Hospice in the United States seeks an experienced Registered Nurse (RN) for a Home Health position delivering skilled nursing care to patients in their homes across the Mifflintown area.

Qualifications

• Active RN license in the state you work
• One year of RN experience
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Deliver skilled, patient-centered care in home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
