Residential Home Health and Hospice is hiring a Registered Nurse (RN) for home health in Wheeling, IL, working 40 hours per week. This role delivers skilled nursing care to patients in their homes, within an interdisciplinary team, with autonomy and meaningful patient relationships.

Qualifications

• Active RN license in the state you work.
• One year of RN experience delivering patient care.
• Must have driver’s license and be able to spend ~20% of day driving to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
