Job summary

Residential Home Health and Residential Hospice is seeking a Home Health RN to provide skilled nursing care in patients' homes around Elgin, IL. The position offers a $10,000 first-year bonus and a 40-hour work week with travel within the assigned territory.

You will collaborate with an interdisciplinary team, perform assessments, manage medications, and develop care plans. Strong EMR documentation and patient-family education are essential.

Qualifications

• Active Registered Nurse license in state you work
• At least 1 year of RN experience delivering skilled nursing care
• Valid driver’s license; ability to travel to patient homes (~20% of day)

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Think critically, solve problems in real time, and ensure thorough EMR documentation

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
