Job summary
At-Heart Home Care & Hospice in Virginia Beach, VA is seeking a dedicated Home Health Registered Nurse to join our team. This role offers 1:1 patient care, flexible scheduling options, and opportunities for professional growth within a supportive environment.

Qualifications
- Registered Nursing Degree (Associate or Bachelor) with current unrestricted license in the state of practice.
- One year nursing practice in a patient care setting.
- Valid driver's license and proof of car insurance as per policy.
- New nursing graduates may be considered in select markets based on program availability.
- Current CPR certification.
Responsibilities
- Assess/monitor physical, emotional, and psychological needs of patients.
- Create home health care plans aligned with MD orders and patient goals.
- Direct nursing care: administer medications, treatments, and interventions.
- Provide pain and symptom management.
- Educate and support patient's family and caregivers.
- Collaborate with an interdisciplinary team and maintain timely documentation.
- Participate in on-call rotation as required by the local branch.
