Job summary

CommonSpirit Health at Home is seeking a Home Health RN to lead in the patient’s home, delivering high-quality nursing care and implementing individualized care plans. You will collaborate with a multidisciplinary team, educate patients and families, and mentor support staff to ensure cohesive care.

A valid RN license in the state served and CPR/BLS certification are required, with at least 1 year of nursing experience, preferably in home health.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Interpret and apply clinical policies to home health care.
• Demonstrate exceptional critical thinking and case management abilities.
• Maintain CPR/BLS certification.
• Reliable transportation and valid driver’s license.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care in the patient’s environment.
• Develop and adjust comprehensive care plans to promote independence and well-being.
• Collaborate with physicians, therapists, and support staff to ensure holistic care.
• Educate patients and families, providing teaching and emotional support.
• Oversee and mentor LPNs, LVNs, and Home Health Aides to build a cohesive team.
