Adoration Health is seeking a Home Health Registered Nurse to join our Clarksville, TN team. You will provide 1:1 patient-centered care in the home, assess needs, develop care plans, and collaborate with an interdisciplinary team to achieve outcomes.

Qualifications

• RN degree from an accredited college with current license in the state of practice
• At least 1 year nursing practice in a patient-care setting
• Valid driver’s license with clean driving record and vehicle insurance
• New nursing graduates may be considered in select markets
• Current CPR certification

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans aligned with MD orders and patient goals
• Direct nursing care: administer medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support patient’s family and caregivers
• Collaborate with an interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by the local branch
