A leading healthcare provider is seeking a compassionate Home Health Registered Nurse (RN) to provide care to patients in their homes. Responsibilities include coordinating patient care, performing assessments, and collaborating with healthcare teams. Pay: $57 to $62

Hours: 40 hours a week 5 8 hour shifts
Contract length: 13 weeks
We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide quality care to patients in the comfort of their homes. The RN will coordinate patient care, perform assessments, administer medications, educate families, and collaborate with a multidisciplinary team to support each patient’s recovery and independence.
Perform comprehensive patient assessments in the home setting.
Develop and implement individualized care plans in collaboration with the physician and care team.
Monitor patient progress and adjust care plans as necessary.
Administer medications, treatments, and wound care per physician orders.
Educate patients and caregivers on disease management, medication usage, and home safety.
Document all care provided, observations, and patient responses accurately and timely.
Coordinate with other healthcare professionals including physical therapists, occupational therapists, and social workers.
Ensure compliance with federal, state, and agency regulations.
Provide emotional support and guidance to patients and families.
Participate in case conferences and staff meetings.
Current, unencumbered RN license in the state of practice
Minimum of 1 year of clinical nursing experience (home health experience preferred)
BLS (Basic Life Support) certification required
Valid driver’s license and reliable transportation
Excellent communication, organizational, and critical thinking skills
Proficient with electronic medical records (EMR)
Frequent travel to patient homes
Ability to lift or move patients if necessary
Exposure to various home environments and potential allergens
