Your Role as an RN Case Manager:
  • Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families.
  • Provide hands-on care, administer medications, and evaluate patient progress.
  • Maintain and submit timely documentation, including OASIS assessments.
  • Collaborate with therapists, caregivers, and healthcare professionals to ensure comprehensive care.
  • Educate patients and families on treatment plans and self-care techniques.
  • Inform patients of additional services and resources as needed.
What You Need to Succeed:
  • Active RN License in Pennsylvania
  • BLS Certification
  • Valid Driver’s License & Reliable Transportation
  • Strong Assessment & Documentation Skills
  • Ability to Work Independently & Manage Time Effectively
  • Excellent Communication & Interpersonal Skills
  • Acute Care and/or Home Health Experience (Preferred)
