Job summary

Emblem Healthcare in the Central Phoenix Area seeks an experienced RN Case Manager to build relationships with patients and families, coordinate care, and create personalized care plans that improve outcomes.

You will collaborate with an interdisciplinary team and educate patients and caregivers, supported by a system that values growth, mentorship, and professional development.

Qualifications

• Currently licensed as a Registered Nurse in Arizona.
• Minimum of 2 years nursing experience, preferably in Home Health, Emergency, ICU, or MedSurg.
• Strong clinical skills and a passion for patient-centered care.
• Ability to work independently and make a real difference. Driver’s license and reliable vehicle.

*This is a full-time position providing service in the Central Phoenix Area *Annual salary range $85000 - $90000 per year (depending on experience), plus travel expenses.

Requirements
