Job summary

Emblem Home Health & Hospice is hiring a full‑time RN to deliver Start of Care visits, assess patients, and build personalized care plans that enhance independence. You will manage wound care, administer IV therapies, and document OASIS accurately while coordinating with the care team.

Qualifications

• Currently licensed as an RN in Arizona.
• Minimum of 1 year Home Health preferred.
• Wound care and/or infusion experience preferred.
• Strong clinical skills and a passion for patient-centered care.
• Expert organizational and time management skills.
• Current CPR certification.
• Valid AZ driver's license, proof of auto insurance, and reliable transportation.

Responsibilities

• Perform Start of Care (SOC) patient visits, including initial physical assessment, medication reconciliation, home safety evaluations
• Create personalized care plans to improve patient outcomes and support their independence
• Perform wound assessments and provide wound care treatments if necessary
• Administer and monitor IV/infusion therapies per physician orders when required
• Complete accurate and timely clinical OASIS documentation
• Act as a trusted Case Manager, coordinating care with physicians, therapists, and the interdisciplinary team
• Educate and empower patients and caregivers with confidence
