Job summary

MercyOne Home Care & Hospice – Mason City, part of Trinity Health, seeks a Registered Nurse (RN) to deliver high-quality home care. You'll perform assessments, create care plans, administer meds, educate families, and coordinate with the interdisciplinary team. Schedule: Monday–Friday 8am–4:30pm with weekend rotation; on-call Saturday-Sunday.

Iowa RN license required; BSN preferred, ADN accepted. Valid driver's license and reliable transportation.

• BSN preferred; ADN accepted.

Responsibilities

• Perform comprehensive patient assessments and identify care needs.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Administer medications and treatments, monitor patient responses, and adjust care as needed.
• Educate patients and families on disease management, medications, and wellness strategies.
• Monitor vital signs and manage chronic conditions with timely interventions.
• Accurately document all care activities and maintain compliance with healthcare regulations.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Provide emotional support and guidance to patients and their families.
• Participate in ongoing education and quality improvement initiatives.
