Job summary

MercyOne Home Care & Hospice – Mason City is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated home care team. You will deliver high-quality, personalized care to patients in their homes, helping them maintain independence and improve their quality of life. Monday–Friday 8am–4:30 pm; weekend rotation based on how many nurses are employed.

On call only Saturday and Sunday 8am-4:30 pm on your scheduled weekend. No weekday or weekend evening call.

Qualifications

• Active RN license in Iowa.
• BSN preferred; ADN accepted.
• Home health/community nursing experience is a plus.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and manage time effectively.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive patient assessments and identify care needs.
• Develop and implement individualized care plans with interdisciplinary teams.
• Administer medications and treatments, monitor patient responses, adjust care as needed.
• Educate patients and families on disease management, medications, and wellness strategies.
• Monitor vital signs and manage chronic conditions with timely interventions.
• Document all care activities and maintain regulatory compliance.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Provide emotional support and guidance to patients and families.
• Participate in ongoing education and quality improvement initiatives.
