Job summary

Cozad Community Health System is seeking a Home Health & Hospice Registered Nurse to perform patient home visits and develop a nursing plan of care that aligns with our philosophy and goals. The RN will serve as primary nurse/case manager coordinating with the PCP and delivering direct patient care in home settings.

Responsibilities

• Serves as the primary nurse/case manager for all assigned patients.
• Develops the plan of care for each patient, in coordination with the primary care provider.
• Oversees that plan of care for effectiveness.
• Assesses patient’s condition at each home visit.
• Documents assessment accurately and reports to the physician as needed.
• Follows HIPAA guidelines and promotes patient safety.

Carries out physician orders, dressing changes, venipuncture, home infusion, medication reconciliation, etc. as specified in the plan of care.Performs and maintains documentation.Follows all HIPAA guidelines and promotes and ensures patient safety.Fulfill mission statement by performing job duties with a high level of customer service while contributing to a positive and professional environment.Special projects and other duties as assigned.Education and/or ExperienceGraduate of an accredited school of nursing. Previous Home Health & Hospice experience preferred.
