Job summary

The Home Health & Hospice Registered Nurse makes home visits and develops a nursing plan of care and implements that nursing plan of care to patients at their place of residence in accordance with the philosophy, goals, and objectives of Cozad Community Health System.

Graduate of an accredited nursing program with Nebraska RN licensure; home health/hospice experience and EHR proficiency preferred. Valid driver's license required for in-home visits.

Qualifications

• Graduate of an accredited school of nursing.
• Nebraska RN licensure required.
• Home Health & Hospice experience preferred.
• Electronic Health Records experience preferred.
• Basic Life Support (CPR) required; ACLS preferred.

Responsibilities

• Serve as primary nurse/case manager for assigned patients.
• Develop and oversee the plan of care with the primary provider.
• Perform home visits, document assessments, and report to physicians.
• Carry out physician orders and ensure patient safety in accordance with the plan of care.

Develops the plan of care for each patient, in coordination with the primary care provider, and oversees that plan of care for effectiveness.* Assesses patient’s condition at each home visit. Documents that assessment accurately and reports to the physician as needed.* Provides direct patient care and demonstrates knowledge in home care/hospice as well as scientific and technical principles, drugs, and therapeutics. Carries out physician orders, dressing changes, venipuncture, home infusion, medication reconciliation, etc. as specified in the plan of care.* Performs and maintains documentation.* Follows all HIPAA guidelines and promotes and ensures patient safety.* Fulfill mission statement by performing job duties with a high level of customer service while contributing to a positive and professional environment.* Special projects and other duties as assigned.## Qualifications**Education and/or Experience**Graduate of an accredited school of nursing. Previous Home Health & Hospice experience preferred.
